Most joint supplements failed their big trials. Fortify skips glucosamine entirely and uses the two ingredients that did not. What that is worth.

Fortify leaves out glucosamine and chondroitin, which is the right call and an unusual one β€” the largest trial of those two found no benefit over placebo for most people. What is left is UC-II collagen and curcumin, both of which have small positive trials and neither of which is a cure. At $1.67 a serving this is a considered formula in a category full of unconsidered ones.

The trial that emptied this category

The GAIT trial put glucosamine and chondroitin against placebo in roughly 1,600 people with knee osteoarthritis. Neither beat placebo in the overall group. Those two ingredients are still the backbone of most joint supplements on the shelf, because the category sells on familiarity rather than on the result. Fortify contains neither, and that omission is the most informative thing on its label.

What UC-II actually is, and why 10 mg is not a typo

Undenatured type II collagen works by a different route from the collagen powders sold for skin. It is not building material; it is thought to act on the immune response in the joint, which is why the studied dose is 40 mg of material carrying about 10 mg of the active collagen rather than the grams you would take of a peptide powder. The trials are small β€” a few dozen people each β€” and mostly on knee discomfort during exercise. Real, but modest, and not a treatment for arthritis.

Curcumin, and the reason the form matters

Plain curcumin is barely absorbed. Meriva is a phospholipid complex built to fix that, and it is the form several of the pain trials used. The evidence for curcumin on joint pain is more consistent than for most of this category, which is a low bar cleared. If your joints hurt from training, this is the ingredient most likely to do something you notice.

What no supplement here replaces

Load management. Most training joint pain is a volume or technique problem, and no capsule fixes a programme that is asking too much of a tendon. Take the supplement if you want the small edge; fix the programme first, because that is where the effect size lives.

Questions

Why is there no glucosamine in it?
Because the largest trial of glucosamine and chondroitin found no benefit over placebo for most participants. Leaving it out costs the product a familiar name on the label and is the better decision.
How long before it does anything?
The UC-II trials ran for 90 to 180 days. Curcumin trials on pain are shorter, often 6 to 12 weeks. Nothing here works in a week, and a product promising that would be lying.
Is collagen powder the same thing?
No. Collagen peptide powders are hydrolysed and taken in grams as building material. UC-II is undenatured and taken in milligrams for an immune-mediated effect. They are different mechanisms and the doses are not interchangeable.
