Why This Happens
The mechanism is unglamorous, which is probably why it gets so little attention.
These medications do not block absorption. Nothing about them interferes with how you take up iron or vitamin B12. The problem is upstream: you are eating substantially less food, and micronutrients arrive inside food.
A person eating 1,300 calories instead of 2,400 is, all else equal, taking in roughly half the vitamins and minerals they were. Over a few weeks that is irrelevant β the body has stores. Over a year of continuous treatment it stops being irrelevant.
Two things make it worse. Protein takes priority when volume is limited and correctly so, which crowds out the vegetables and fruit that carry the widest micronutrient range. And gastrointestinal effects can make specific food groups unappealing for weeks at a time.
Pooling STEP 1β3 (semaglutide 2.4Β mg, n=2,117; placebo, n=1,262), the most reported gastrointestinal events were nausea (43.9% vs 16.1%), diarrhoea (29.7% vs 15.9%), vomiting (24.5% vs 6.3%) and constipation (24.2% vs 11.1%). 99.5% of events were non-serious and 98.1% mild-to-moderate, occurring most often during or shortly after dose escalation.

Eating less food means eating fewer micronutrients, which is the mechanism this page describes. A multivitamin covers gaps; it does not fix a deficiency, and a diagnosed one is a question for a doctor and a blood test.

Which Nutrients Go First
Not everything is equally vulnerable. The ones that matter here are those with limited stores, high requirements, or narrow food sources:
- Iron. Limited stores, and red meat β the densest source β is commonly one of the first things people go off.
- Vitamin B12. Animal-source only. Stores are large, so depletion is slow, which is exactly why it is missed until it is significant.
- Vitamin D. Already widely insufficient before anyone starts a medication, and food contributes little.
- Calcium. Concentrated in dairy, which often falls when portions shrink.
- Folate, magnesium, potassium. Carried mostly by vegetables and legumes β the bulky foods a suppressed appetite accommodates least.
The Diagnostic Problem
Deficiency symptoms are fatigue, poor concentration, hair loss, brittle nails, feeling cold, low mood. Every single one of those is also attributed to the medication, to the caloric deficit, or to weight loss generally.
So people experience them, conclude βthis is the drugβ, and wait. That is the failure mode β and it is the same overlap that hides dehydration and fatigue.
The only way through it is measurement. You cannot reason your way to whether your ferritin is low.
Bottom line: If tiredness or hair loss appears months into treatment rather than around a dose change, deficiency moves up the list of explanations.
What To Ask For
A reasonable panel to request, particularly at six months and then annually:
- Full blood count β picks up anaemia from several causes
- Ferritin and iron studies β ferritin depletes before haemoglobin falls, so it catches it earlier
- Vitamin B12 and folate
- Vitamin D (25-hydroxy)
- Thyroid function β not a deficiency, but it produces the same symptoms and needs excluding
Baseline assessment before starting is better still. Joint guidance already recommends baseline measurement of strength and body composition, and the same logic applies here β without a starting value, a later result is much harder to interpret.
A joint advisory from four major U.S. nutrition and obesity societies sets a protein target of 1.2β1.6Β g/kg body weight per day (or about 1.5Β g/kg of lean body mass) during active weight loss, distributed at roughly 25β30Β g per meal, and recommends structured resistance training alongside pharmacotherapy.
Read the advisory βDensity, Not Volume
The instruction βeat more vegetablesβ is useless when volume is the binding constraint. The useful version is to raise nutrient density per bite.
- Red meat and liver for iron and B12 β small portions carry a lot.
- Eggs β protein plus a broad micronutrient range in very little volume.
- Dark leafy greens β cooked rather than raw, since cooking collapses the volume dramatically.
- Legumes β protein, fibre, folate and magnesium together.
- Dairy or fortified alternatives for calcium.

When A Supplement Is Justified
The honest position: a multivitamin is not a substitute for food, and taking one does not make an inadequate diet adequate. But when intake is pharmacologically suppressed for months, insisting on food-only is a position, not a plan.
The order that makes sense:
- Measure. Supplementing a deficiency you have is useful; supplementing one you do not is expensive urine.
- Correct specific deficiencies specifically. Iron in particular should be dosed on a result, not taken speculatively.
- A general multivitamin as insurance is reasonable during sustained low intake, and cheap.
Important: Iron is the one not to self-prescribe. Excess iron accumulates and causes harm, and the symptoms of overload overlap with the symptoms of deficiency. Test before supplementing, and let a clinician set the dose.
