Most Common Side Effects (GI)

Gastrointestinal effects dominate the side-effect profile of every drug in this class, by a wide margin. In order of how frequently they're reported across trials:

The mechanistic explanation for the gallstone signal is straightforward: rapid weight loss itself — regardless of how it's achieved — is an independent, well-established risk factor for gallstone formation, and GLP-1 therapy produces weight loss fast enough to plausibly compound that risk on top of any direct drug effect.

Hair loss

Hair shedding (telogen effluvium) is a commonly discussed concern, but the stronger explanation in the current evidence is the rapid-weight-loss trigger itself — a well-documented cause of temporary hair shedding independent of any specific weight-loss method — rather than a direct pharmacologic effect of GLP-1 receptor agonism.

Fatigue

Reduced caloric intake, possible micronutrient gaps, and reduced fluid intake are the leading plausible explanations; this isn't a headline finding in the pivotal trials so much as a commonly self-reported experience layered on top of the caloric deficit itself.

Rare but Serious: Know the Red Flags

Pancreatitis is a listed rare risk in FDA prescribing information for this drug class. Pooled trial and postmarketing surveillance data have not established a clearly and consistently elevated causal risk across the class, but it remains an actively monitored signal — severe, persistent abdominal pain (especially radiating to the back) warrants immediate medical attention, not a wait-and-see approach.

Hypoglycemia (low blood sugar) is uncommon on GLP-1 monotherapy specifically, because these drugs stimulate insulin release only when blood sugar is already elevated. Risk rises meaningfully when combined with insulin or sulfonylurea medications — a combination that requires closer glucose monitoring, set by a prescribing clinician.

Why These Side Effects Happen

Nearly every effect on this list traces back to the same core mechanism: GLP-1 receptor agonism slows gastric emptying and gut motility. That's a feature, not a bug — it's a major contributor to the appetite suppression and blood-sugar control this drug class is used for. But the same mechanism that keeps food in the stomach longer (supporting fullness) is what produces nausea, and the same slowed gut transit that reduces snacking is what produces constipation.
